By far the most common water heater type found in American residential housing is a gas or electrically heated storage tank type. These are relatively simple appliances and generally operate successfully day-in and day-out with little maintenance or thought. However, like any appliance, they eventually fail and often an engineer is needed to explain why.
As "water hammer" (i.e. liquid impact) has the potential to cause glass bottle failure, Read Consulting LLC created two such failures to study the resulting fracture surface details.
A tornado can cause catastrophic and extensive damage to all types of structures. For some, the damage requires a total rebuild. Plaintiff was admitted to a health care facility with ventricular tachycardia (VT). He had a normal coronary angiography with moderate left ventricular (LV) function and was referred for an implantable cardiac defibrillator (ICD) implant procedure. A diagnosis of right ventricular outflow tract tachycardia was made. He was then admitted for an ablation procedure in March 2005, which was successful. In April 2005, he was readmitted with a further occurrence of tachycardia which required emergency cardio version due to hemodynamic compromise and a temporary pacing wire was placed. There was a further attempt at ablation that was again successful and the LV function noted now to have deteriorated and was noted as having poor functioning. On this basis he was considered a candidate for an ICD implant. There was a further reoccurrence of an episode of ventricular tachycardia (VT) in March and April 2006 which was treated medically and he was discharged.

It is essential when beginning a root-cause failure analysis involving metallic materials to collect and maintain the as-found physical evidence properly prior to detailed examinations. This has two objectives. In all cases collecting and protecting the evidence in its undisturbed condition provides the best opportunity to derive meaningful good results from the subsequent analyses. Secondly, in a legal proceeding lack of attention to these matters can produce the accusation of spoilation of evidence. As a minimum this will cause presentation problems for the offending party in the suit. At worst such deficiencies by that party may cause the case to be thrown out.

To evaluate the effect of residual stresses on rail integrity, an analysis technique was sought that could quantify the effect of these stresses on web crack propagation.

Recently, the street mall pavers have experienced a substantial number of loose and cracked granite pavers. Atkinson-Noland & Associates conducted an extensive investigation at the 16th Street Mall granite tile pavers to determine the causes of distress in the paving system. We also helped develop appropriate solutions to paver debonding, slip-resistance, aesthetics, and maintenance.
Washing machine failures can create significant damage, exposing insurers and their clients to property damage. These failures often involve a high volume of water and can occur without warning. Through the examination of several hundred washing machines, the DONAN Component Testing Laboratory has identified a statistically significant failure trend in General Electric (GE) washing machines. The amount of water supplied to the tub of the washing machine is controlled by a water valve (solenoid-controlled valve) and pneumatic controls. The washing machine's load selector is a pneumatic pressure switch, which activates and deactivates the water valve.
