    Air bag control modules utilize complex algorithms to make air bag deployment decisions based on crash severity related to the change in vehicle speed or deceleration over time. Due to the proprietary nature of air bag deployment algorithms, the velocity, acceleration, or displacement thresholds for air bag deployment during a collision are not easily obtained; however, a range of frontal barrier impact speeds and corresponding deceleration and displacement threshold values for air bag deployment can be approximated using known vehicle stiffness-to-weight ratios.

    All is quiet at 10:30 pm until a nurse notices water on the 2nd floor hallway. Building maintenance is called and discovers a ruptured pipe fitting in a bathroom on the 2nd floor. The water is shut off at 10:45 pm but unfortunately the damage has been done. It is later discovered that water penetrated through perforations in the ceiling in the CT suite on the 1st floor exposing the 16-slice CT gantry and patient table to water.

    A "gauger" employed by an oil production company was going by boat to different well locations to get the wells flowing. In other words, although he was performing oil field related activities, he was a seaman and the gauger was employed in consideration of his seamanship abilities because to perform the duties required by his employment he was obligated to spend the majority of his time aboard vessels and/or on marine structures and operate vessels without assistance regardless of weather or water conditions.

    Wakes are moving waves created by a boat or vessel when advancing across the water. A ship moving through the water is accompanied by at least three "pressure disturbances" on each side, which produces several trains of waves. This wave system is characterized by: a). divergent waves; b). transverse waves; and c). a line of crest intersections (cusps) where the divergent and transverse waves meet.

    During a very nice seven-day cruise on a large passenger vessel in the Caribbean, a lady passenger decided to use a coin operated laundry room, available aboard the vessel, which was available to the passengers. The washers and dryers were heavy-duty stackable units, with the washers resting on the deck or floor of the laundry room, and the dryers mounted on a steel frame just above the washers. After drying the clothes just washed, the passenger was pulling clothes out of one dryer when the dryer fell on her resulting in serious injures.

    The accident took place in an open ocean anchorage area (blue water) off the coast of Colombia, about 5 miles offshore. At this location, bulk carrier vessels are moored to buoys and floating cranes are used to transfer coal from barges to the oceangoing bulk carriers.

    Some types of accidents are well known because despite of the continuous efforts to reduce them, they continue happening, such as slip and fall accidents. Others are less common but are easy to prevent or to minimize their consequences. The following is a sample of an offshore accident that could have easily been prevented.

    A lady passenger aboard a cruise ship was walking in the proximity of the Purser's desk, in a "Public Space" (interior passageway), when she tripped and fell to the floor hitting also a fire control door, sustaining serious injuries. The fall was caused by tripping on a large metallic fire screen door threshold, which was protruding substantially above the carpeted floor

    This is a sample of numerous accidents investigated by the author of this article. The assistant driller on a Jack-up Rig owned and/or operated by an Offshore company, was performing various tasks on the drilling floor, when, one of the floor hands (roustabout) had to leave the drilling floor and the assistant driller replaced him.

    This 48-foot 66 MPH luxury power cruiser was beautiful inside and outside, but during trials and during the test ride, a squeaking "humming noise" was noticed in the proximity of the icemaker cabinet door. The frame of the cabinet door was attached to the hull and the salesman convinced the potential buyer that the noise would disappear after the boat was operated a few hundred miles. According to the salesman, there were built-in residual stresses that will be released by operating the boat with motions and slamming for several hours. The boat was sold despite of the squeaking.

    Dr. J. P. Purswell, PhD, PE, CPE has extensive experience in Human Factors, Ergonomics and Safety Engineering. He holds a doctorate in Industrial & Systems Engineering from Virginia Tech. A member of the Human Factors and Ergonomics Society and the American Industrial Hygiene Association, Dr. Purswell served as the past Chair of the Industrial Engineers PE exam from 2005 to 2009. He continues to serve on the committee which is responsible for the preparation of the national professional engineering examination in industrial engineering.

    Dr. Purswell is the author of more than 20 publications and presentations on Product Safety, Occupational Safety, Printed Warnings, and Auditory Warnings. Examples of products addressed include silica, asbestos, cleaning chemicals, gasoline containers, and recreational equipment. Dr. Purswell maintains NIST-calibrated equipment for the measurement of light, sound, force, and whole-body and hand-arm vibration in addition to standard photo and video equipment.

    Dr. Purswell has been an Expert Witness in the legal industry since 1997. He has been qualified as an expert in state and federal courts across the nation in safety engineering, including safety analysis techniques such as Fault Tree Analysis and FMEA as well as product warnings / instructions and OSHA compliance issues. Dr. Purswell's services include site inspections, document review, drafting Rule 26-compliant reports, deposition, and trial testimony. He has been retained by attorneys for both Plaintiff (40%) and Defense (60%) more than 100 times, has been deposed approximately 50 times, and has testified at trial over 20 times.

    In 2005 a summary of "General Duty Clause" citations issued for ergonomic hazards was published (Purswell & Purswell, 2005). That summary showed that the primary area of ergonomics citation activity under the "General Duty Clause" (paragraph (5)(a)(1) of the OSHAct of 1970) by OSHA to that point had been concentrated in nursing homes, peaking in 2002 and 2003.

    The purpose of the current study was to update and expand upon an earlier study performed to review and categorize OSHA accident investigation records for pedestrian-backing vehicle accidents according to whether the backing vehicle had a backup alarm and whether the alarm was installed and functioning as intended. The current study includes an analysis of additional records as well as the business type (SIC code) of the employer.

    Richard Hughes, PhD, specializes in the disciplines of Biomechanics and Ergonomics. He is an Associate Professor in the Departments of Orthopaedic Surgery, Industrial & Operations Engineering, and Biomedical Engineering at the University of Michigan.

    Dr. Hughes is director of the Laboratory for Optimization and Computation in Orthopaedic Surgery (LOCOS), which is housed in the Department of Orthopaedic Surgery at the University of Michigan. His research currently focuses on Bayesian modeling in spinal biomechanics and quality improvement in hip and knee arthroplasty.

    Following the completion of his doctoral studies and prior to his current role, Dr. Hughes worked at the Washington State Department of Labor and Industries, National Institute for Occupational Safety and Health, and the Mayo Clinic.

    Litigation Support - Dr. Hughes offers expert witness services to attorneys representing both Plaintiff and Defense.
