    Torsional problems of the femur have been traditionally treated by a proximal osteotomy with internal fixation. We elected to perform femoral derotational osteotomies distally. Between September 1994 and April 2001, supracondylar osteotomies were performed on 38 femora in 21 children with torsional and angular deformities. The average age was 9 years (range 5-15 years). Twenty-three femora had excessive anteversion and fifteen, retroversion. All osteotomies were maintained by the small AO external fixator.

    Patients with fixed equinus and associated angular and rotational deformities, who have had multiple previous surgeries, present a significant challenge to the orthopaedic surgeon. We chose to correct these deformities with supramalleolar extension wedge osteotomies in 21 feet in 13 patients between 1991 and 2002. The median age at presentation was 11 years (range: 2-17 years). An average correction of 20" of extension (range: 10-33") was required to achieve a plantigrade foot. Fourteen of 20 feet (70%) remained plantigrade at a mean follow-up of 6 years.

    On June 14, 2013, the nation paused to pray and remember the unspeakably horrific shooting, a massacre that claimed 26 lives, on December 14, 2012, at the Sandy Hook Elementary School, in Newtown Connecticut.     Isn't it interesting how the courts recognize taint in so many different contexts?1 We have an illegal search by the police and everything found during the illegal search becomes "fruit of the poisonous tree"-it is said to be tainted. The Houston Police Department's Crime Lab does not observe proper sterile procedures in handling DNA and the specimens are contaminated-they are said to be tainted. We have a lineup where the crime victim sees one White guy, three Hispanics, and two Blacks. The identification of the White guy is said to be tainted, the product of a suggestive or improper lineup.

    When divorce occurs in the family, an inevitable loss occurs in the lives of the parents as well as children. Positive adjustment to divorce comes when both of the parents as well as the children develop healthy coping methods to deal with the loss. This enables both parents and children to move forward with their new lives. Unfortunately, many parents who suffer from their own disturbed attachment history or mental health issues, are unable to put aside their anger and constructively cope with the loss. The perpetrator and not the victim is given this diagnosis. The victim is given an abuse diagnosis.

PTSD in children and teens is caused by events that have caused them or someone else to be killed or badly injured. Not all children develop PTSD after a traumatic injury. 3-15% of girls and 1-6% of boys develop PTSD in response to a traumatic experience. Rates of PTSD are higher for certain types of trauma survivors. Nearly 100% get PTSD if they see a parent being killed or if they see a sexual assault. 90% of sexually abused children develop PTSD; 77% of children who see a school shooting develop PTSD; and 35% of children who see violence in the area they live, develop PTSD (National Center for PTSD in Children and Teens).

    Amputation is an injury involving loss of function, loss of sensation, and loss of body image. It is not surprising amputees often suffer psychological difficulties following such an event. Further, these psychological difficulties can also result in long term Post Traumatic Stress Disorder (PTSD) for the amputee. Often these psychological injuries and resultant PTSD can be difficult to explain to a jury. The forensic expert's presentation of psychological testing assessing potential PTSD is one part of proving or disproving damages. However, the forensic expert can better assist either the Defense or Plaintiff's attorney by addressing specific functions in the amputee that have been affected by the injury.

    Wilderness therapy, aimed primarily at adolescents presenting with problem behaviors, includes a range of interventions that incorporate nature and the wilderness as prominent aspects of treatment. In theory, wilderness therapists seek to effect both behavioral and attitudinal changes. Through providing a context of challenging outdoor living and a group setting in an emotionally safe place, wilderness therapy aims to address problem behaviors and to facilitate an emerging sense of healing, well-being, and personal and social responsibility.1 Wilderness therapy's action-oriented approach is said to be particularly appropriate for adolescents and is intended to build self-esteem, internal locus of control, interpersonal skills, trust and team building, and a sense of group belonging.2

    The sensationalism of many criminal trials, especially those of a capital nature, often result from the aggravating circumstances impacting the victim.2 Conversely, the mitigating circumstances that affect the accused's criminality rarely grab headlines.3 During the sentencing phase of a criminal trial, mitigating factors may justify leniency or otherwise serve to lessen the sentence for the crime with which the accused has been charged.4 Whether a particular factor will be considered a mitigating one will depend upon the particular facts of the case. The federal Sentencing Reform Act of 19845 provides guidance in this process,6 but each state maintains the discretion to dictate its own criteria within the confines of constitutional constructs.7 The United States Supreme Court has increasingly addressed the importance of clarity in the presentation and consideration of mitigating evidence, which is integral to the trial and sentencing of an accused.8

    Officers throughout the United States and perhaps internationally have heard use of force instructors discuss the "21 Foot Rule" during their officer safety, firearms and deadly force training. As both a use of force instructor and practicing forensic police practices expert, I have also trained and testified to this concept myself.

    On a hot July day, fire and police are called to the home of a 55 year old man suffering from heat stroke. Police arrive first and find the man sitting on a bench in his front yard. When the officers approach the man and ask him to give them his cane, he becomes agitated and non-compliant. One officer suddenly grabs the cane away from the man, who screams and suddenly stands up. The man is tased, taken to the ground, beaten and handcuffed. He is transported to the hospital where ER physicians confirm a diagnosis of heat stroke. The man has no criminal history. He sustains numerous moderate injuries which keep him from returning to work for several weeks. The officers and agency are sued and settle out of court.
