Stephen L. Nelson, MD, PhD, FAAP, is a dual board certified Child Neurologist with over 15 years of experience in Pediatric Neurology and Pediatrics.

Dr. Nelson is the current Director of Pediatric Neurology at Tulane Medical Center in New Orleans, LA, where he performs high volume inpatient and outpatient evaluations. He runs the pediatric epilepsy center which offers video EEG monitoring as well as epilepsy surgery. Dr. Nelson is also a full-time Associate Professor of Pediatrics, Neurology, Neurosurgery, and Psychiatry at Tulane University School of Medicine. He holds a medical license in the states of Louisiana, Mississippi, and Maryland and is certified as a medical doctor expert witness by the state of Florida.

    From the days of "Reefer Madness" until the timing of Proposition 19 with the "Legalization of Marijuana Initiative" on the ballot last November, the controversies swirling around not only the recreational use, but also the medical use of marijuana, particularly in frail, elderly geriatric patients with clinically significant weight loss (and cachexia), has been fraught with controversy. Even though the ballot initiative from 2010 went down in defeat on November 2, 2010, rumblings of some of the authors bringing another, more "targeted" CA initiative in 2012 are regaining momentum.

    A disturbing trend has emerged involving criminal charges being levied against family caregivers treating end-of-life (EOL) elders with dementia.

    The willingness to view risk as part of daily life has vanished. A risk-averse mindset among environmental regulators engenders confusion between the ethics of intention and the ethics of consequence, leading to the elevation of the precautionary principle with unintended and often unfortunate outcomes. Environmental risk assessment is conservative, but the actual level of conservatism cannot be determined. High-end exposure assumptions and current toxicity criteria from the USEPA, based on linear extrapolation for carcinogens and default uncertainty factors for systemic toxicants, obscure the degree of conservatism in risk assessments. Ideally, one could choose a percentile of the target population to include within environmental standards, but this choice is complicated by the food, pharmaceutical and advertising industries, whose activities, inadvertent or not, often promote maladaptive and unhealthy lifestyle choices.
