Dr. Nicholas Ladikos is a Clinical Pharmacist and Drug Regimen Error expert. He is a subject matter expert and serve as expert witness in determining the standard of care and breaches of that standard on matters such as medical chart reviews, medication preparation and prescribing, drug side effect management, and errors in drug dispensing, dosing, and administration. 

Dr. Ladikos also serves as a Professor at both Texas Tech University and Arizona College of Nursing. He co-authored numerous papers in the areas of infectious diseases and antimicrobial stewardship, pharmacoeconomics, pharmacogenomics, geriatrics, and therapies in the lung transplant population. He conducts physician training and clinical staff education regarding medication trends and utilization, identification and mitigation for potential medication fraud, waste and abuse, and collaborates in disease management and population health programs.

Litigation Support - Dr. Ladikos is a subject matter expert in several pharmacy-related areas and serves as an expert witness on a number of Pharmaceutical litigation matters. Numerous hospitals have used his skillset and expertise in medical chart reviews to identify errors in medication preparation and prescribing, drug side effect management, standard of care compliance, continuing education delivery, and personnel assessments.

Dr. Ladikos currently develops and leads clinical programs that help stakeholders use and advance medication management tools (e.g., formularies, pharmacy, therapeutics committees and medication therapy management), quality metrics, research methodologies, and health information technology. He has experience in medication utilization statistical analyses to demonstrate adherence to evidence-based practice guidelines and inform health program policies.

Dr. Ladikos examines pharmacoeconomic trends to ensure that a cost-effective pharmacy benefit is administered to members, and consults with care management team to provide guidance regarding member goals. 

Megan M. Hamilton, MS, CCC-SLP earned her Master of Science degree from Marquette University and holds a lean six sigma green belt from Purdue University as well as a Certificate of Clinical Competence from the American Speech Language and Hearing Association (ASHA).  Megan serves as a forensic medical SLP throughout the United States.  Megan has held positions as direct care provider, site manager, regional vice president, and division manager of clinical education for national rehabilitation providers in long term care, sub-acute, and pediatric care.   She brings over 40 years of experience to her clients.  

Litigation Support - No healthcare provider can prevent aspiration and/or choking for all patients at all times, but all can be expected to practice within accepted standards of care.  Megan's exhaustive review of case materials can determine whether diagnosis, evaluation, recommendations and discharge determination were based upon sound clinical reasoning to mitigate adverse outcomes and facilitate improvement or if they were substandard.  Megan provides evidence-based opinions whether care contributed to negative outcomes including suspected choking event, aspiration, dehydration, health decline or death or if it did not.

Wayne H. Grant, PharmD, is a Clinical Pharmacist with over 30 years of pharmacy experience working in or within various practice settings—retail, long-term care, home infusion, sterile products, corrections, hospice, specialty, and hospitals.  

Dr. Grant has prepared and delivered educational programs to physicians, nurses, and other healthcare professionals based on his extensive experience in multiple disease states. Programs include topics such as Pain Management, COPD, and Motor Neuron Diseases.

Serving as an integral member of multiple pharmacy and therapeutics committees, he has promoted evidence-based interventions while driving quality initiatives and building a highly collaborative consortium among healthcare professionals.  

Earning his Doctor of Pharmacy from the University of Florida, his Master of Business Administration for Kent State University, and his Bachelor of Science from Ohio Northern University highlights Dr. Grant's drive for education, understanding, and application of pharmacy services.

David Friedler, MS, MPH, CNHA, LMHC, Is an expert in Wrongful Injury, Abuse and Neglect in Health Care. His experience is in the regulatory standard of care in Nursing Facilities, Psychiatric and Substance Use Rehabilitation as well as Adult Foster Care. He holds MPH and MS degrees and has worked clinical and administrative roles for four decades. He has extensive experience with survey compliance, Federal and State regulation, JCAHO and CARF accreditation, OSHA compliance and policy and procedure adherence.

Dr. Hoffman is a Geriatric Neuropsychologist in Northern California with a practice focusing on the assessment of capacity, undue influence, conservatorships, & elder abuse in older adults. She specializes in the differential diagnosis of dementia and their effect on capacity.

Dr Hoffman serves as an expert witness and has testified on issues of capacity and undue influence. She is the author of “The Assessment of Testamentary Capacity & Undue Influence in the Older Adult,” in Forensic Geropsychology (2018).
